Create Survey | New Survey - How to?
How do I create a new Survey?
Login » Surveys
Click on Create a New Survey Button.
You can select from one of the three options and click on the Continue button
Copy a Survey Template
New Survey (From Scratch)
Import Word Document
To create a survey from scratch select the New Survey (From Scratch) option and click Continue to start the Survey Create Wizard.

Step 1: Enter the survey name and select the folder. Click on the Next button.

Step 2: Select the Theme for the survey and click on the Next button. If you wish you can finish and exit the wizard on this step.

Step 3 (Optional): On this step you can set up survey authentication via Social Networking. When enabled users will be asked to log in to facebook before taking the survey.

The Survey will be added and you can proceed to adding questions.
